Forms:

  • The following forms are available on the Cassell Training Center website.
  • Please print, complete, and bring forms with you to registration.
             Registration Form
             Liability/COVID Form
             Concussion Form
             Debit Card/Credit Card/ACH Payment Authorization Form
             Photo Release

PLEASE ONLY 1 PERSON IN BUILDING FOR REGISTRATION

Payments:
  • Payments required at Registration:
  • There is a 3% charge if using a debit or credit card.
  • Late fees for declined credit card is $10.00
  • $25.00 charge to for insufficient funds from checking and $10.00 late fee 
           
 Class Information (Class list is found on website):
  • Please note on the Class Description List that some classes will require teacher approval for Registration.
